Travis Came home from the Hospital today.... weight 4 1/2 lbs.
His stay in the hospital was 2 mos. and 4 days.
Travis had an eye examination done by Dr. Greenberg that revealed retinopathy of prematurity, stage 1, zone 2. He will be followed by Dr. Greenberg as an outpatient. Travis was discharge from the hospital with a apnea home monitor, and a bilateral inguinal hernia, that will be repaired after a few months old as an outpatient, Travis continued gaining weight. On caffeine, he did not have any more apnea and bradycardias.
November 23, 1990 Dr. Morgan Pediatric Surgery 1200 S. Kuhl ave. Orlando Florida 32806
Travis had his hernia repeared. Doctor Morgan wanted him to but on more weight but we had to move the surgery up. After the surgery Doctor Morgan said the scrotal edema and echymosis will be resolved within the next two weeks, and unless there are problems he will not specifically schedule him back for a follow-up vist.

January 24, 1991 Arnold Palmer Hospital, had ultrasound on head and chest x-ray done at A.P. hospitial. Amoxil 75mg. 10 days. for EARS
B-mode echoencephalogram.
The head appears elongated in the AP diameter and some what narrow transversely, but the underlying brain appears normal.

This is the last visit to Dr. Finer's office. Did not like him.(He's a QUACK)
March 14, 1991 Developmental center visit report.
Study done at 5 3/4 months, corrected age 3 3/4 months. Study done at orlando regional medical center. EXAMINATION: Length is 10 percentile, weight 75 percentile, and head circumference 90 percentile. His head shape is dolichocephalic with bitemporal narrowing. There is a relatively prominent forehead. His eyes are deeply set, but otherwise normal. His chest is clear to auscultation. No murmur is appreciated at the present time. He received phototherapy for hyperbilirubinemea and required red cell transfusion for anemia. Travis has some respiratory problems, but was discharged home without any significant complications. He did require an apnea monitor and remains on this. Travis had a hernia repair performed in December. His external genitalea appear normal. There is residual fluid present in the scrotum.
Developmentally, Travis is social. His hands are kept open most of the time. He responded to noisemakers. Travis scored at a functional age of 3 1/2 months on the mental developmental portion, and the same on psychomotor developmental portion.

Travis now six months old. His weight is 14 lbs. Height is 22 1/2 inches.
Quadruplet B Travis has had 2 ear infections in the past. Has apnea monitor, and has been receiving caffeine 1cc per day. Parents have been alarmed that he has been having lots of apnea alarms. Yesterday he alarmed at 12 midnight and at 3 a.m. Parents even changed monitors thinking there was a monitor problem but he continued to have these alarms. Parents are concerned about the baby's head size. Although he is markedly below the 5% in height he is between the 50% in head circumference and his head appears the largest amonst the 4 quads. Will notify Dr. Chiaro at the apnea center regarding the apnea alarms.
Has tiny umilical hernia. Has bilateral hernia scar. Circumcise male testes descended. Hips abduct well. has good head control.

Spoke with Dr. Chiaaro regarding Travis's recent apnea episodes. We are going to do a sleep study tonight. Then a event study lasting 7 days. Increase his caffeine to a dose of 5 mgs per day.

Short apnea was seen on occasion but none of these episodes exceed 6 seconds' duration. The average heart rate was 125 to 100 beats per minute.

Dr. Pollack saw Travis at 6 month-old because of macrocephaly. This is his report. Father indicates he was the largest of the quadruplets at 2 lbs. 13 oz., and remained in the nursery for 2 and 1/2 months but required only a few days of respirator support. Intraventricular hemorrhage did not occur. He was discharged on an apnea monitor. Father's record indicates that 2 alarms sounded in November and 5 in March.
Physical examination -includes a weight of 15.5 lbs. and a head circumference of 44 cm. There is dolichocephaly and frontal bossing. Anterior fontanel is patent. The anterior portion of the cranium appears expanded relative to the posterior portion. Chest is clear except for scattered rhonchi. There is no hepatosplenomegaly. Genitalia are normal. A herniorrhaphy scar is present. The patient's head circumference falls above the 98th percentile for his corrected age. Travis's head is elongated in the AP diameter, and that there is frontal prominence. Neurological Examination: Sagittal synostosis with macrocephaly. CT scan and physical examination show no evidence of brain compromise resulting from the patient's sagittal synostosis although risk of cerebral compromise does exist with continued growth. Cosmetic effects of the sagittal synostosis will also worsen with time if the patient"s condition is not treated. Three-D CT scan of the head was ordered to better visualize the synostosis. Dr. Montoya will see Travis within the next few days and I anticipate that linear craniectomy will be performed in the near future.

Travis has premature closure of sutures and will be seeing Dr. Montoya for consultation in the morning. Has been scheduled for 3 dimensional x-rays next monday 4-1-1991. Travis had 3 apnea alarms on 3/20/91 in a 24 hour period and that makes 7 apnea alarms for this month. Will start 2 week event study after which we will more than likly increase his caffein dosage.

Swelling and redness on right arm. Has had small cyst a month ago which was seen by a plastic surgeon Doctor Sweitzer that his brother Cory goes too. Surgeon thought it to be benign. It is markedly red and tender and the size of a dime, may need to be drained. Travis is due for surgery for craniosynostoses, however this is postponed due to infection and neosurgeon wants this completely treated before surgery.

Dr. Montoya was able to successfully repair the craniosynostoses. (premature closure of sutures of Travis head.) The surgery lasted 6 hours. A 1/2 inch strip of bone was removed sagittally. Dr. helmling the urologist did another circumsion. The first one was done in the hospital. At the time of the surgery Travis's wieght was 3 lbs. His penis was to small so it had to be revised.--------(Circumcised again.)

A 1/2 inch of bone was removed sagittally and already the head configuration looks a little more normal with an increase in the parietal diameter, The distance between the 2 sides of the bone measures 1 1/2 inches and this wide open area appears soft and not tense or bulging.

Pt age 7 months has trouble breathing all day, respiratory rate noted by nose was approximately 64 per min. And has been wheezing since evening. Will call in with progress report. Pt will check in at the hospital. Phone call after 2 hours. Pt's respirator rate increased to 80 and after Ventolin now is 54 per min. He is sleeping and is much more comortable. 2 a.m. pt left for home. Father will call back if ther is any worsening during the nite. office visit in the morning.

Came in for having trouble breathing last night called in Ventolin for bronchiolitis. His respiratory rate has ranged from 50-80. CBC done at office reveals white count of 5.3, Platelet count 416,000. Cont. on Ceclor and Ventolin. Recheck in 5 days.

Despite the ventolin pt. continues with rapid wheezing respirations. Father had pox occimeter at home (for his quad with BPD) and it read 99% Later in the day respirations became more labored and the pox occimeter read 92%, Father gave him some nasal 02 and he seemed to be more comfortable. Dispense home nebulizer pulmo-aid. Use Ventolin solution for inhalation. insteda of giving the Ventolin orally, Father is to use the pulmo-aid nebulizer treatment, Give us a progress call tomorrow. Cont. ceclor.

Very much improved since seen last time. Wheezing is audible only when he cries now. Respiratory rate is in the 40's and totally unlabored. Lungs are clear when he is not crying but when crying he has expiratory wheezes. BSOM, bronchospasm. T-gram confirms flat curves bilaterally. Start bactrim. Cont. Ventolin with nebulizer. Lotrimin apply to the rash in the body folds which has taken on more of a fungal apperance, report if not improving. Today received DPT #2, OPV #2, and hip #3.

Had 1 apnea alarm which occurred after the end of a very busy day swimming at the neighborhood pool, etc. Father express some concern about caffeine making him "stare off into space" or at times get irritable with slapping his hands against other people or against his own face. Pt weighs 2 more pounds than he did when we started him on the higher dose caffeine in
and since is weaning himself from the calculated dose. Has an appt. with Dr. Montoya for a postop visit following his cranial stenostosis surgery, and I want him to mention this to Dr. Montoya and see how concerned he is about it. ( In the menatime, should cont. the AB monitor.)

Head circumference paralleling growth curve between 50 - 75%. He is in the 25 % in weight and just at the 5% in height. Can pull himself to a standing position, has beensitting alone since age 9 months. No teeth yet. Had some apnea alarms in the past week with prompt correcting on his own. Just completed his bedtime prophylaxis on bactrim last week. Cont. Bactrim and return at age 12 months. Has and appt. with Dr. Montoya and should mention the apnea alarms that he has beenhaving to see if they relate at all to his cynostosis surgery. Today received DPT #3, HBOC #3. and Hip #3.

Dr. Tulipan: On examination after moving to Tennessee, Travis has fairly mild scaphocephaly. The sagittal suture is already ossified except in one small area. Given this re-ossification, he may continue to have a progressive cranial deformity in the future. At the moment, His deformity is mild enough that I certainly would not intervene. We'll see him back in about six months for follow-up. Craniectomy for sagittal Craniosynostosis.

Chest x-ray from Donelson showed a patchy right middle lobe infiltrate with hyperexpansion.
Hospital course: He was started on nebulizer treatments 3 hours which were changed to 4 hours by a. m. He received a loading dose of Prednisone and Amoxicillin for the pneumonia.
Discharge: He was discharged the a.m. after admission to continue home asthma therapy. Medications Albuterol, Amoxicillin, and Prednisone.
DISCHARGE DIAGNOSIS: Asthma exacerbation and Pneumonia.

Emergency Department Report.This 2-year-old male who is onset of a set of Quadruplets, presented to the E-R with complaint of fever, by the father. The patient has been undr treatment by Dr. linden for ear infection, has been on Ceclor for 10 days. Dr. Linden saw the patient 3-4 days ago and thought that the ear was progressing well. He has, in addition to the Ceclor, also been placed on ear drops. Today the father noted the fever. He has also noted 2-3 loose stools. He did not classify those as watery diarrhea, but says that the stools were loose, and this has been over the past 2 days. Patient has some drainage from the ear. He comes to the E-R for further evaluation.
Review of systems: The patient is allergic to Bactrim. Medications include ceclor, Tylenol, ear drops, Ventolin. The patient has a past medical history of ear infections with ear tubes, asthma and pneumonia times three.

November 4, 1994 I took Travis to Dr. Wyatt for his Phenomena shot. I hope I did the right thing, it took seveal days for the Doctor to get the shot and then I asked the nurse what are the side effects and the nurse said I don't know I never gave this shot to anyone, do you want to know before I give it to Travis. I said yes plaese.
